    ELEVEN COPIES OF THIS APPLICATION, SITE PLAN ( DRAWN TO SCALE, 8.5" X 11" IS ACCEPTABLE), AND A LETTER TO THE BOARD STATING THE REASON FOR REQUEST ARE REQUIRED. ONE COPY OF PROOF OF OWNERSHIP MUST ACCOMPANY THIS APPLICATION. YOU MAY ADD ANY OTHER DOCUMENTATION (PHOTOS, ETC.)

    When submitting a site / sketch plan, please indicate all dimensions of the property, buildings, etc., and any other information necessary for the appeal. It is strongly recommended that a property survey be used.
